This terms and conditions document is to accept your request for surgery and make a legally enforceable agreement without further reference to you. You must read these Terms and Conditions to ensure they contain all you want and nothing you are not happy with. If you are not sure about anything, phone us on 0800 015 0885.

These terms and conditions contain important and relevant information about us and how we will provide you with care service.

General Information

Our website provides a comprehensive overview of our services. Regarding services offered based on your request, you must ensure the accuracy of any information or specifications you provide.

The registration form and the Terms and Conditions form the primary agreement, the ‘Agreement,’ between you and Eagle Eye Care. In the event of any conflict between the registration form and these terms and conditions, the terms will be given priority. The agreement becomes effective upon signature.

It may be necessary to update this agreement periodically. You will be notified of any changes using the contact information you recently provided. You will always be able to review any revised agreement and ask questions if necessary. You are solely responsible for the reviewed treatment costs and the terms and conditions outlined in the new agreement. This agreement will be updated on the website as well.

Expert Advice

Once your examination consultation with our optometrist is complete and you are deemed suitable for the eye operation, you will be invited to attend a complimentary eye examination consultation with your surgeon.

Please be aware that, if necessary, additional consultations with the optometrist before surgery or if you have any concerns and wish to speak directly with the surgeon will be charged associated with these services. The specific charges will be discussed during the telephone or virtual consultation.

As the website outlines, a fee will be applied for the initial consultation. Kindly be informed that Eagle Eye Care reserves the authority to assess prices, which will be promptly communicated and reflected on our website for your convenience.

During the initial consultation, we will thoroughly discuss all the details regarding the services available and determine the most suitable service for your specific eye condition. In addition, details regarding payment, including the method and terms, will be addressed during this consultation phase. At this stage, you will receive a consent form in your email. You should carefully read through the form and provide your signature where required.

The consultant will contact you to discuss the details of your surgery. They can connect with you via video or phone or arrange a one-on-one meeting. This communication will occur within 7 days before your scheduled surgery date(s). The consultant will receive all assessment scans and notes during the initial consultation. If it is necessary for you to visit the eye clinic, we will schedule a consent appointment for that purpose.

Following that, you will receive a treatment decision form. At this point, you will be requested to sign a consent form. Nevertheless, in the event of a disagreement and your decision to decline treatment following the initial consultation, Eagle Eye Care may apply reasonable administrative charges for conducting the consultation, as outlined in the information provided during the booking process.

Payment

To secure the booking of your surgical procedure, a minimum deposit of up to £500 is necessary. Please ensure the remaining payment is made 7 days before your surgical procedure. Off-setting the balance 7 days before your surgical procedure is essential. Failure to do so may result in the cancellation of the process, and the deposit paid will be non-refundable. You can use the deposit to reschedule to a different date. Please note that the rescheduling of the date needs to be done within 3 months from the original cancellation of the surgery. Rebooking is limited to a single occurrence and cannot be transferred to another patient.

During the initial consultation, the optometrist will provide a comprehensive overview of all the costs. You can find all the information regarding eye care and surgery costs in the price list provided in the treatment section of our website. The initial consultation is typically offered at no cost. Nevertheless, suppose you decide not to proceed with the treatment after the complimentary initial consultation. In that case, Eagle Eye Care may apply reasonable administrative charges for conducting the consultation based on the information provided during the booking process. If you are ready to move forward after the initial consultation, you can proceed to book any treatment. You will be accountable for any expenses related to your treatment, and all payment details will be given to you during or after the initial consultation. Please ensure that payment for your treatment or appointment is made in cleared funds no later than 7 days before your scheduled date. You will be responsible for covering the hospital’s prescriptions and consumables expenses.

You must inform us if you choose to stop receiving ongoing treatment. Charges for treatment that you have received up until the point that you notify are charged in full, but you will not be charged after you decide to cancel or reschedule a treatment less than 72 hours before the due appointment. If it exceeds 72 hours and you choose to cancel or reschedule treatment, you will be charged a late cancellation/non-attendance fee.

On any occasion, we make decisions regarding your treatment and discover that you may no longer be required to receive any ongoing treatment from us. We will let you know as soon as possible and explain our decision. We will charge you for all treatments, private prescriptions, and consumables you have received until we notify you. No charges apply after this point. You are not obligated to continue receiving ongoing treatment from us. We will let you know as soon as possible and explain our decision. There are no obligations required to discontinue treatment once you start. However, you may be advised of the risks involved. Eagle Eye Care will not be held responsible for any costs incurred in attending your treatments, such as transportation or accommodation.

After you are discharged from our post-operative care, we recommend that you arrange yearly eye examinations. You are responsible for taking these examinations and must pay any applicable charge. Alternatively, patients undergoing lens replacement or cataract surgery may also attend a refractive surgery follow-up consultation, which involves more in-depth diagnostic tests than a standard eye examination.

Suppose unforeseen circumstances arise and decisions are made regarding your treatment such that you can no longer receive ongoing treatment. In that case, you will be informed as soon as possible and backed with reasons. You will be charged for all treatments and consumables received until you are notified. No charges apply after this point.

Alternative dates and times for treatment will be provided. On occasions where it is impossible to reschedule your treatment, you will be refunded any amount paid for the treatment.

Payment plans are available, which gives patients the advantage of spreading the payment across a specific period. The payment plan will be discussed during the initial consultation, and a consent form containing the terms and conditions for such a payment system will be provided accordingly.

Overdue debts will be referred to a debt collection agency as approved by Eagle Eye Care. At that stage, you may become liable for interest and the cost of recovering the debt. Eagle Eye Care reserves the right to add collection costs and interest for any services provided to any overdue account.

Payment Cancellation and Returns

Before the initial consultation, if you cannot attend your appointment, we kindly request that you notify us at your earliest convenience. Please feel free to cancel or reschedule your appointment by calling 0800 015 0885.

After booking a treatment, you have a 7-day cooling-off period to cancel the treatment without any charges. Nevertheless, any treatment received during the seven (7) day cooling off period will require payment. Additionally, if you decide to cancel or reschedule a treatment within 24 hours of the appointment, we may charge you 25% of the treatment price as a late cancellation/non-attendance fee. Eagle Eye Care is committed to providing you with a refund for any excess amounts already paid to cancellation or non-attendance fees. Kindly be advised that Eagle Eye Care cannot assume responsibility for any expenses related to attending the treatment, including transportation or accommodation. Refunds will not be provided for such costs.

If your ophthalmic surgeon suggests an alternative surgery option, such as a multifocal lens instead of a mono-focal lens, there could be an additional cost. This additional payment is required in addition to the previous amount paid. You have the authority to decide whether to continue or discontinue the treatment. If you choose not to proceed with the suggested surgery, you can cancel and receive a refund for your initial payment.

If your surgery date is changed within 21 days of your planned surgery, there will be a charge of £…… per eye. No fees will be associated if the date is modified more than 21 days before your scheduled surgery.

Special Offers – Laser Eye Surgery or Refractive Lens Replacement Surgery

Discounts can be applied to the cost of laser eye surgery or refractive lens replacement surgery for both eyes, offered by Eagle Optical Limited. This offer applies explicitly to the surgery cost, ensuring a clear focus. This offer excludes Cataract Surgery, Laser eye surgery, and refractive lens replacement. These surgeries are governed by Eagle Eye Care’ standard terms and conditions. When you reach out to us through our website or by phone, we can provide you with a copy via mail. Any promotional discount offered applies to both Laser Eye Surgery and Refractive procedures. The promotion will remain valid if Eagle Eye Care cancels the laser eye surgery consultation or refractive lens replacement surgery consultation due to circumstances beyond the presenter’s control and then reschedules.

In addition, please note that the promotion cannot be combined with any other offer. It is also important to mention that the promotion is non-transferable, and no cash alternative will be provided. Furthermore, it is worth noting that the price match guarantee and 0% interest-free credit cannot be used in conjunction with any monetary discount promotion. The promotion is limited to one person at a time.

Unfortunately, we do not offer any monetary discounts for entry-level procedures such as LASEK/PRK, Monofocal Lens Exchange, and Cataract Surgery.

Eagle Optical Limited can modify or end any monetary discount promotion.

Discounts are redeemable against the cost of either laser eye surgery or refractive lens replacement surgery done on both eyes provided by Eagle Optical Limited. The offer applies to the cost of surgery only. The offer does not apply to Cataract Surgery, Laser eye surgery, or refractive lens replacement. These surgeries are subject to Eagle Eye Care’ standard terms and conditions. You can obtain a copy through your mail when you contact us using our website or by phone call 0800 015 0885. Any promotional discount is valid for either Laser Eye Surgery or Refractive. The promotion will be valid if Eagle Eye Care cancels the laser eye surgery consultation or refractive lens replacement surgery consultation at no fault of the presenter and subsequently re-booked.

Also, the promotion cannot be combined with any other offer; it is non-transferable, and no cash alternative will be offered. Price match guarantee and 0% interest-free credit cannot be used with any monetary discount promotion. The promotion is only applicable to one person at a time.

Monetary discounts are unavailable for entry-level procedures, including LASEK/PRK, Monofocal Lens Exchange, and Cataract Surgery.

We intend to promote our services to you, and you always have the option to unsubscribe or opt out whenever you wish.

We kindly request that you provide us with your telephone number and other necessary information. These details enable us to effectively communicate with you and assist our experienced advisers in scheduling appointments and addressing any inquiries, concerns, or legal matters that may arise. We may also contact you if you are in our database of potential patients or leads and have chosen to receive communications from us or have not opted out. Kindly be aware that all calls to our Customer Engagement Centre are recorded to ensure the highest level of quality assurance.

This applies to all individuals or organisations involved in billing, payment processing, collection, or credit referencing activities. To accomplish this, Eagle Optical will implement the necessary measures to safeguard your data.

Eagle Optical is required to share specific clinical data with governmental and regulatory bodies, such as the General Medical Council (GMC), the General Optical Council (GOC), the Nursing and Midwifery Council (NMC), the Medical and Healthcare Products Regulatory (MHRA), and the Care Quality Commission (CQC), as mandated by law or regulation. This may involve any personally identifiable clinical information, such as your NHS patient identification number or equivalent.

Retention of Data

This only applies if a more extended retention period is necessary or permitted by law or to meet a legal obligation. We will retain your personal information for the appropriate duration.

As part of our commitment to maintaining high standards, we must keep a record of your eyesight test results for a minimum of 10 years.

Suppose you are interested in receiving laser or lens treatments with us. In that case, we kindly request that you attend one of our clinics for a consultation to assess your suitability for surgery. During the consultation, we will collect the necessary information, such as your contact details, health history, and lifestyle, to evaluate if surgery suits you. Additionally, we will conduct an eye examination to assess your vision.  The information gathered during your laser or lens consultation or treatment is essential to your medical record. It must be kept for a minimum of 10 years, as mandated by our regulators.
