PATIENT TERMS AND CONDITIONS FOR CATARACT

Cataract Surgery: A surgical procedure that involves removing the cloudy lens and replacing it with an artificial intraocular lens (IOL) to restore clear vision
Patient: The person who is undergoing lens replacement surgery at Eagle Eye Care.
Surgeon: An ophthalmologist licensed to perform lens replacement surgery.
Eagle Eye Care: A healthcare facility that performs lens replacement surgery.
Eligibility and pre-surgical Requirement
A comprehensive eye assessment is necessary for all patients to assess their eligibility for cataract surgery. Complete disclosure of pertinent medical history and current medication usage is essential to guarantee the safety and effectiveness of the procedure.
Informed Consent
Patients will be provided with comprehensive information on the surgical procedure, which will include extensive explanations of the anticipated results, possible risks, advantages, and alternative options. Patients are advised to enquire and deliberate on any enquiries or apprehensions they may have with the ophthalmologist. Prior to performing the procedure, it is necessary to get written informed permission from the patient.
Procedure Descriptions
A proficient ophthalmologist will conduct the cataract surgery utilising cutting-edge technology and techniques. The choice of surgical approach and lens implants will be tailored to meet the unique requirements of each patient.
Risks and Adverse Effects.
Cataract surgery, like any other surgical operation, has inherent risks such as infection, irritation, and visual issues. Eagle Eye Care will offer an extensive inventory of potential hazards throughout the consultation procedure.
Care and monitoring after surgery and subsequent check-ups
Following post-operative care recommendations is essential for a good recovery. Subsequent meetings will be arranged to oversee the progress of healing and attend to any difficulties or concerns. Patients are advised to promptly notify medical professionals of any atypical symptoms or problems that arise subsequent to the surgical procedure.
Charges and Remuneration
A comprehensive cost estimate, encompassing the expenses for surgery, lens implants, and post-operative care, will be given before the operation. The user's text is a bullet point. Conflict Resolution

We appreciate you deciding on Eagle Eye Care for your lens replacement treatment. This document outlines the terms and conditions that will govern the surgical procedure and your interactions with our clinic. Before surgery, it is critical to thoroughly read and comprehend these agreements. They define your legal rights and responsibilities as a patient, as well as your healthcare provider's obligations.
Definition
Lens Replacement Surgery: A surgical procedure that replaces the eye's native lens with an artificial intraocular lens.
Patient: The person who is undergoing lens replacement surgery at Eagle Eye Care.
Surgeon: An ophthalmologist licensed to perform lens replacement surgery.
Eagle Eye Care: A healthcare facility that performs lens replacement surgery.
Eligibility and preoperative evaluation
Eligibility Requirements: To be eligible for lens replacement surgery, patients must undergo a thorough medical evaluation and ocular examination. This includes an evaluation of the patient's overall eye health, vision requirements, and any pre-existing conditions that could influence the outcome of the surgery.
Pre-surgery assessment: The pre-surgery examination consists of a comprehensive eye exam, as well as measures of the eye's interior anatomy. Medical history assessments include any pre-existing conditions and medications. Visual requirements and expectations are assessed to ensure that the procedure aligns with the patient's goals.
Patient Responsibility: The patient is responsible for providing an accurate as well as comprehensive medical history and informing Eagle Eye Care of any changes in their health status before the surgery. If important information is not disclosed, the surgery's outcome may be affected, and complications can result.
Informed consent: Patients will be receiving information regarding the procedure, expected outcomes, and potential risks as determined by the assessment.

Eagle Eye Care will provide post-operative care and follow-up. Post-operative care instructions. Patients are obligated to attend scheduled follow-up appointments to monitor their recovery.
Terms of Payment
Unless a payment plan has been arranged with Eagle Eye Care, the patient will be advised of the cost of the surgery, including any additional fees, prior to the the procedure. Payment must be made in full prior to the surgery date. Cash, credit/debit cards, and approved insurance plans are all acceptable payment methods.
Exclusion
Eagle Eye Care is not liable for complications resulting from noncompliance with pre- or post-surgical instructions. These terms do not cover cosmetic outcomes, changes in visual acuity, or the natural ageing process, which can impact vision. Pre-existing conditions that have not been disclosed prior to surgery may be excluded from coverage.
Conclusion of Care
The patient's care is considered complete when they have completed the surgery, followed all post-operative instructions, and attended all required follow-up appointments. Subject to additional fees, the surgeon may administer additional remedies or corrective interventions as necessary.
Privacy and Information Security

Procedure for lodging a complaint
Patients who have any concerns or complaints about their care or treatment at Eagle Eye Care are advised to address them immediately with their surgeon or a clinic's management staff member. Written complaints can be sent to Eagle Eye Care's administrative office or emailed to contact@eagleeyecare.co.uk. We will respond to all complaints within seven days of receiving them. Eagle Eye Care will undertake a comprehensive investigation into the complaint and provide written clarification within 30 days. If a complaint cannot be resolved to the patient's satisfaction, the patient can request a review by an impartial arbitrator. Further information about the arbitration procedure will be provided upon request.
Conflict Resolution
Any conflicts emerging from the cataract surgery will be resolved through arbitration or in line with the legal system of the jurisdiction. Eagle Eye Care will make every endeavour to settle issues in a friendly and efficient manner.
Cancellation and Refund Policy
To avoid cancellation fees, patients must provide Eagle Eye Care with at least seven days' notice if they would like to cancel or modify their procedure. For cancellations made lesser than three days before the surgery, a fee may be imposed to cover the costs involved with the operation's preparation and execution. Eagle Eye Care's return policy states that prepayment refunds will be granted, excluding any applicable fees. Refunds will be processed within 14 days of the cancellation. If you are compelled to rescind for medical reasons, you may be required to provide documentation from a medical professional.
Acknowledgement
By signing this agreement, you acknowledge that you have read, understood, and agree to the terms and conditions set forth above.

PATIENT TERMS AND CONDITIONS FOR YAG SURGERY
By submitting your deposit, you acknowledge and consent to abide by these Terms & Conditions. This is applicable even if you have not formally agreed to these Terms and Conditions.
1. Remuneration
The booking of your surgical procedure requires a minimum deposit of 500 pounds. The remaining amount must be settled seven days before the scheduled date of your surgical treatment. Failure to pay the remainder seven days before the scheduled surgical operation will result in the cancellation of the procedure and forfeiture of the entire deposit. The deposit can be applied to the reservation of the new date for your surgical procedure. If the rescheduled date falls within three months from the original booking date for the procedure, the deposit can only be applied to one rebooking. It cannot be transferred to another patient's booking.
The price for your primary surgical operation encompasses the expenses of:

All eye drops and prescriptions an Eagle Eye Care Clinician recommends within the first three months following your initial surgical treatment.

A YAG Capsulotomy is a non-invasive laser therapy that can eliminate foggy vision that may occur due to your initial treatment if an ophthalmic surgeon from Eagle Eye Care suggests this within the initial 12 months following your primary operation.

You will have eight follow-up appointments, which can be done in person or by telemedicine, as advised by an optometrist or ophthalmic surgeon within the first 12 months after your initial surgery.
Suppose an optometrist from Eagle Eye Care and an ophthalmic surgeon determine you are suitable for a laser eye surgery enhancement. In that case, we will offer you one procedure per eye free of charge. This offer is only valid if the enhancement procedure is performed within the first twelve months after your initial surgery. If it is clinically essential, we often evaluate the possibility of an augmentation surgery 3-9 months after your initial surgical procedure
Nevertheless, depending on your clinical requirements, the ophthalmic surgeon will determine the appropriate timing. In the unlikely event that an additional surgical procedure or treatments are required for a specific eye after the initial laser surgery and enhancement, you will be responsible for any associated procedure fees. Please note that this does not apply to fixed-price cataract surgery.
You are accountable for any additional appointments following your participation in 8 aftercare consultations within 12 months of your primary surgical procedure.
After the primary surgical procedure is completed for 12 months, you are responsible for the cost of additional surgical procedures or remedies.
If your optometrist or surgeon (or both) determines it is appropriate, your postoperative care will conclude, and you will be discharged.
We suggest you schedule annual eye examinations after discharge from our postoperative care. Arrangements for these examinations are your responsibility, and any applicable fees must be paid. In contrast, patients who have undergone cataract or lens replacement surgery may also participate in a refractive surgery follow-up consultation, which entails more complex diagnostic testing than a standard eye examination.
You are accountable for the cost of any additional eye drops or prescribed medications after your primary surgical procedure after the first three months.
You are accountable for any additional appointments following your participation in 8 aftercare consultations within 12 months of your primary surgical procedure.
After the primary surgical procedure has been completed for 12 months, you are responsible for the cost of any other surgical procedures or remedies.
If your optometrist or surgeon (or both) determines it is appropriate, your postoperative care will conclude, and you will be discharged.
We suggest you schedule annual eye examinations after discharge from our postoperative care. Arrangements for these examinations are your responsibility, and any applicable fees must be paid. In contrast, patients who have undergone cataract or lens replacement surgery may also participate in a refractive surgery follow-up consultation, which entails more complex diagnostic testing than a standard eye examination .
2. Modifications to the nature and date of your surgery

There may be an additional charge if your ophthalmic surgeon suggests a different type of surgery than the one you have scheduled, such as a multifocal lens instead of a mono-focal lens.

The decision to proceed with the alternative surgical procedure that has been suggested is entirely up to you. Before the surgery can proceed, you will be required to pay the additional cost if you choose to proceed. If you opt not to undergo the alternative procedure, you may terminate your surgery and receive a refund for the amount you paid, as outlined in the refund policy below.

A charge of 150 pounds per eye will be imposed if your surgery date is altered within 21 days of the scheduled date. If your surgery date is changed more than 21 days before the planned procedure, there will be no additional fee.
3. Expenses associated with your primary surgical treatment:
If your vision isn't perfect after surgery, you may need glasses or contacts, which may be required for additional operations or treatments.

Eyeglasses or contact lenses may be needed after surgery or for vision issues unrelated to the surgery.

Presbyopia, an age-related eye disease that impairs near-vision, may require glasses or contacts. This is only applicable to implanted contact lens surgery.

Any subsequent surgery with a different goal than your initial surgery (e.g., to improve near eyesight).

Any surgery or treatment not covered by your initial surgery. Cosmetic surgeries like eyelid surgery.

Any dry eye treatments like LipFlow therapy or punctal plugs not covered by your original operation.
Dry eye treatments include warming masks, eyelid wipes, or Omega 3 supplements.
An appointment or medical procedure conducted by a healthcare professional who is not an optometrist or ophthalmic surgeon and is not affiliated with an Eagle Eye Care center.
There is a possibility that your surgical procedure or postoperative treatment could be disrupted due to equipment failure or the absence of your optometrist or ophthalmic surgeon. If such an incident occurs, we may arrange for your surgical operation or postoperative therapy to be rescheduled at a different Eagle Eye treatment location.
A surgery for corneal cross-linking is conducted.

Immediate surgical interventions or alternative treatments, as your optometrist or surgeon recommends. If advantageous, we may have emergency surgery, treatment, and follow-up care provided at a separate Eagle Eye Care facility, another Eagle Eye Care surgeon, or an independent company or surgeon not associated with Eagle Eye Care. Suppose the ophthalmic surgeon concludes that proceeding with your operation would not be advantageous or believes a different type of eye surgery may be more suitable. In that case, they may decide not to proceed with the planned procedure. If this is relevant, they may suggest you spend more time carefully considering your options.

You have medical issues, such as a severe cold, infection, or an active cold sore (or similar ailment), on the day of the surgery. Considering the present circumstances, it is recommended that you delay your surgical procedure and explore alternative options.

Expenses linked to travel and any additional charges, as well as any income loss resulting from conditions such as taking time off work owing to prolonged healing time for your eyes, difficulties associated with your surgery, or the need for treatment or follow-up care.
Medications prescribed more than three months after your first surgical procedure.
Postoperative follow-up appointments are offered after completing a minimum of 8 sessions or after 12 months since your first surgical procedure. • Any further medical intervention or treatment after an initial procedure to improve a particular eye.
Any surgical interventions, augmentation therapies, or other essential therapy needed more than 12 months after your initial surgical operation. This exclusion applies if you are deemed medically unsuitable for additional surgery, an augmentation procedure, or any other type of therapy within 12 months after your initial surgical operation. This exemption also pertains to medical care received more than 12 months before your initial surgical operation.
4. Overall observation

We cannot guarantee that you will be able to have a treatment or procedure performed by a healthcare professional of the same gender. Suppose you have concerns but still proceed with your scheduled surgery or treatment. In that case, your clinic manager can arrange for a more affordable companion of the same gender to accompany you during the consultation or procedure. You must request the clinic manager make the necessary arrangements on your behalf.

It is important to note that specific medical conditions make individuals ineligible for refractive therapy. These conditions may include, but are not limited to, the use of certain drugs, pregnancy, and breastfeeding. This indicates that if you are considered clinically unsuitable for additional surgery, an enhancement procedure, or any other treatment within 12 months of your initial surgical procedure, you may not be eligible to receive the laser eye surgery enhancement procedure for each eye free of charge during the first 12 months following your initial procedure.
The prices we offer are subject to occasional changes. We will uphold the pricing provided to you at your pre-surgical consultation on the condition that you schedule your surgery within 14 days of the consultation. If we decrease our pricing for your operation after your pre-surgery consultation, we will bill you based on the reduced price at the time of your procedure.
No refunds will be issued if the price of your treatment is reduced after your surgical procedure.
We will utilize personal information about your treatment to facilitate the provision of your treatment, conduct any supplementary procedures related to your treatment, and effectively manage our business operations (e.g., maintaining accurate records and ensuring quality control). • Information concerning your treatment will be employed for research purposes, statistical analysis, and in conjunction with academic and scientific papers, presentations, and other publications. The treatment-related information we utilize for these reasons will not disclose your name.
5. Unanticipated event
A force majeure incident is anything that happens that neither party could have predicted or prevented. Anything beyond our control, such as a war, terrorist attack, earthquake, storm, government action, pestilence, epidemic, or pandemic, might be considered a force majeure event.
If force majeure prevents or delays a party's performance of its duties under this contract, neither party is liable for failing to perform those obligations.

In the event of a force majeure, the party unable to fulfill their obligations under the contract must inform the other party as soon as possible, providing detailed information about the event and why it prevented or delayed their performance. • After the force majeure has passed, the affected party must resume their obligations under the contract as soon as reasonably possible.
Neither you nor we will be held responsible for

Any costs, losses, expenses, damages, or refunds related to the contract price during the force majeure, and

Any costs incurred by you as a result of any delay caused by the event of force majeure
6. Procedure for lodging a complaint
Patients who have any concerns or complaints about their care or treatment at Eagle Eye Care are advised to address them immediately with their surgeon or a clinic manager.
The clinic manager will address your concern as soon as possible to the best of their ability. If your complaint does not reside within their purview, the clinic manager will forward it to the relevant department for investigation and response.
Request that our clinical service department investigate if you are dissatisfied with the response you received from your clinic manager about your complaint or if you are uncomfortable bringing up the issue with your clinic manager. You can accomplish this by submitting written complaints to clinicalservices@eagleeyecare.com.
